Camp Registration
Registration has begun for MK Camp!
We have 30 available spots.
Date: June 29 – July 5, 2012.

Step 1
Get Registered! Due by 5/15/2012

Step 2
CAMP PAYMENT AND CAMPER LETTER
The Camp fee is $230.00 for the first camper and $200.00 for additional family members. Don’t forget the costs of transport (20$ for one way or 40$ for round trip). We are asking that you send in your fee to the address listed in the camper letter by May 30th. This will ensure your registration for this year’s camp. We will contact you when we have word that payment has been received.
Step 3
Release of Liability!
We also need a Release of Liability form signed and witnessed. This document does not have to be notarized.
Step 4
PACK YOUR BAGS!
What to bring with you:
• Bedding (sheets, comforter cover or lightweight sleeping bag, pillow)
• Clothing (jeans, shorts, shirts or T-shirts, jacket or sweatshirt)
• Towel, toothbrush/paste, soap/shampoo
• Tennis shoes for outdoor activities, slippers for inside the house, and shoes that
you don’t mind getting wet for canoeing.
• Rain wear
• Hat or cap to protect against sun.
• Swimsuit, towel, sunscreen, sunglasses
• Flashlight
• Bible/pen/notepad
• Insect repellent
• Copy of medical insurance card or medical insurance information

Only thing left after the 4 steps is for you to get to the DPB House and have a lot of fun!
